If a patent qualifies for both patent term adjustment (PTA) and pharmaceutical patent term extension (PTE) under Article 42 of the Chinese Patent Law, must the patentee choose one, or can both periods be added together?

Both systems may be used, and the periods can generally be combined. If the applicant files both PTA and PTE requests, the Patent Office first determines the number of days of PTA and then the number of days of PTE. If only a PTE request is filed, the Patent Office waits until the PTA request deadline—three months from the date of the grant announcement—has expired before determining the PTE period.
